md5 digest uri not set correctly when using srv

See subject, the digest-uri in the md5 sasl response isn't set correctly when using SRV records. This causes logins to fail on recent versions of ejabberd

So...has this been committed yet? It's just not feasible for every downstream user to apply this patch.
